The Clinical Study of the Congenital Megacolon


Abstract
The clinical study was made on 60 cases of the congenital megacolon for 11 years from 1963 to 1973 at Dept. of general surgery, college of medicine, S.N.U. These studies revealed the following:
1) The sex ratio was 11 : 1, of male preponderance. (91.2%)
2) The symptoms manifested itself within 3weeks after birth in majority of the cases.
3) The clinical symptoms were abdominal distention, constipation, vomiting, malnutrition, and diarrhea in order of frequency.
4) Meconium passing or not was an important diagnostic clue. The meconium was evacuated after digital examination or enema in 60% of the total, and it passed spontaneously in remainders.
5) The majority of the cases (95%) were so called short segment type. The total colonic aganglionosis was not seen in this analysis.
6) The definitive surgery was performed in 33 cases, (Swenson 4, Duhamel 27, Soave 2) am-ong which 22 cases underwent the one stage operations, and the others colostomy followed by definitive surgery.
7) The associated anomalies were mental retardation 2, inguinal hernia 2, mongolism 1, poly-dactylia 1, barrel chest 1.
8) The familial occurrence was not observed in this study.
9) All of the infants were normal full term delivery except only one prematurely.
10) Wound infection was the main complication of early stage after definitive surgery, and incontinence, of later stage.
11) The mortality rate of all was 8.3%, and operative mortality was 3.3%.
12) The long term result was good in 85.7% of 21 follow-up cases.
